Patient Questionnaire Project

We have to share as this is a momentous piece of work it’s been so important for the Pro Active Community members to work on.

Charlotte Rowbotham from Surrey and Sussex Healthcare Trust SASH, has just emailed to confirm the sign off has been agreed on the first Easy Read Patient Questionnaire as part of the Pro-Active Community and Active Prospects UK Surrey and Sussex Healthcare NHS Trust focus group.

Charlotte and Penelope Firshman, also from SASH, have seen first hand the importance of co-producing these forms, and have made these workshop groups experts by experience led.
